requirements for submission

Nominating organizations must submit the following documents to support their nomination for the Private Individual Member position in the CWC Board:

1. Nomination Letter 

• Detailing the nominee’s qualifications and the organization’s official endorsement. Please address the letter to the CWC Board Chairperson:

2. Nominee’s Profile 

• A comprehensive summary of the nominee’s background, including professional achievements, leadership roles, and advocacy work in child welfare.

3. Curriculum Vitae (CV)

• A complete résumé outlining the nominee’s relevant experience, affiliations, and contributions to child and youth development.

4. Organizational Profile and Accomplishments 

An overview of the nominating organization, highlighting its child-focused initiatives and programs. This  must include:

• Annual Accomplishment Reports for the past three (3) years of operation.

• Certificate of Accreditation from the Department of Social Welfare and Development (DSWD) or registration documents from the Securities and Exchange Commission (SEC).

5. Clearances 

• Proof of the nominee’s good moral character, including valid clearances from the Sandiganbayan, Ombudsman, and National Bureau of Investigation (NBI).

6. Consent Form 

• A signed document from the nominee indicating their willingness to serve if appointed.
