Medical Packet Information

  • Lavender page - Checklist, cover sheet.

  • Yellow page - OTC Medication Authorization. This page authorizes us to provide OTC medications to your child at camp. OTC medications stay at camp. If your child needs OTC medications while on the trail, complete the ivory form. If you would administer these medications to your child at home, please consider allowing us to do the same in your absence.

  • Salmon page - Self-Administration Authorization. This page authorizes your child to self-carry their medication in their hiking pack. This form is to be used for medications that STUDENTS carry themselves and are in charge of administering without adult supervision.

  • Ivory page - Yosemite Medication Authorization. This page authorizes ADULTS to carry and administer necessary medications to your child during the trip. Please note: If you fill out this page AND the salmon page, you are indicating that both the chaperone and your child need a set of the medication. (This is not usually the case. Check with Amanda Clark if you believe this is needed.)

  • Goldenrod page - Food Allergy Plan. This page alerts us to important food allergies. Please circle the correct dose for the epi-pen if needed.

Do

  • Draw a BIG "X" through any page that does not apply to your child
  • Get a doctor/clinic stamp on EACH relevant page
  • Sign and date EACH relevant page
  • Indicate Yes or No to EACH OTC medication
  • Have your student turn in to the Lawson office ASAP before 12/6

Don't

  • Use white-out or pencil on any page
  • Fill in both the self-administration authorization (salmon) and the medication authorization (ivory) for the same medications UNLESS you need both the student and chaperone to BOTH carry the medication
