Requirements for students entering Kindergarten

• 5 doses of DTaP (diphtheria, tetanus, pertussis) vaccine

• 3 doses of Hepatitis B vaccine

• 2 doses of MMR (measles, mumps, rubella) vaccine

• 4 doses of Polio vaccine

• 2 doses of Varicella (chickenpox) vaccine

• A physical exam within the last 12 months prior to the start of school

All of the above documentation is required to be uploaded during registration or prior to the start of the school year. After the registration period, documentation may be sent to susan.pielech@cumberlandschools.org or faxed to 401-333-1412 (Attn: Nurse).

This is a state requirement and students without the appropriate documentation can be excluded from school if proof of an upcoming appointment (temporary waiver) or religious exemption form is not provided. See forms below.

A school physical is ONLY REQUIRED DURING REGISTRATION or if a STUDENT TRANSFERS from another school district. Afterwards, there is no need to send a copy of a physical examination until middle school. Documentation of flu vaccines is only required in preschool.
