Medication for In School Administration

If a student needs to take any medication during the school day, the medication administration form must be filled out by both the parent and physician. Please see additional specifications below

1. Over the Counter (OTC) Medications: Provide an unopened original bottle of the medication labeled with student name and date.

2. Prescription Medications: Medication needs to be in the original prescription labeled bottle indicating student's name, date of birth, prescribing physician, and the prescribed medication.
