Bias Reporting

Bias Report Forms

Use these forms to report bias that you experience or witness in the school community. Students, parents, families, faculty, staff, or others who are involved are welcome to complete these forms.

Please select the correct form based on the involved parties. Click the button to go to the form.

*PLEASE NOTE: While form responses will be addressed with all possible speed, it is not intended as a crisis response form. If you or someone involved are in crisis at school, find an adult who can help you immediately. If you or someone involved are in crisis outside of school hours (after 3:45pm on school days, or on weekends or holidays), call the CPS Student Safety Hotline: 773-553-3335.

What is a Bias Related Incident?

A bias related incident is language and/or behavior that causes harm which is motivated by bias against a person because of their actual or perceived identities. This can include race, ethnicity, language, religion, gender, sexuality, ability, appearance, or financial status.

EXAMPLES OF BIAS RELATED INCIDENTS:

  • Hate messages or symbols posted on social media

  • A student wears a costume to school that is based on a racial stereotype

  • A teacher refuses to use the correct name and pronouns for a trans student

  • A student mocks the way a classmate with a disability speaks

  • A teacher tells a student that their hair texture is unprofessional and encourages them to straighten their hair

  • A student makes jokes disparaging the neighborhood their classmate lives in

  • A staff member makes repeated comments about a colleague’s weight and body size

Why is this reporting needed?

Bias related harm is unfortunately common, but many students may not feel comfortable reporting bias to an adult at school face to face.

The DEI Committee developed these forms with the following goals in mind:

  • Create more methods for students to seek help and support at school

  • Develop a consistent method of documenting bias at our school

  • Streamline the process for responding to bias related harm to address it in a timely manner

  • Track and assess bias at our school to address trends in the wider community

Many colleges and universities have bias incident reporting forms, which we used a a model when we created the forms for Whitney Young! Check out the University of Chicago or Harvard University Bias Incident Reporting forms to see how these work in a college setting.

How Can Harm Be Repaired?

When bias related incidents are reported with names and specific situations, the school will work to find a way to repair the harm caused. Involved parties and administrators will work together to find the resolution that fits each situation best.

EXAMPLES OF POSSIBLE RESOLUTIONS TO BIAS RELATED HARM:

  • A conflict resolution meeting with involved parties

  • A Restorative Justice Peer Conference to mediate a confidential conversation

  • Disciplinary responses

  • Regular appointments with school social worker or counselor

  • A Town Hall addressing the issue with the wider WY community
