Please make sure to accept your offer online to begin the enrollment process.
Please make an appointment with this link (coming soon) to complete the verification and enrollment process. Enrollment appointments during the week of June 22 are available here.
Enrollment packets may be picked up from the security desk to be completed prior to your appointment or one will be provided during your enrollment appointment.
During this appointment, please bring the following:
Child’s Birth Certificate/Passport
2 Proofs of residence (ID/Drivers' License, Utility bill within 30 days (gas, electric, water))
Immunizations/physical Forms (if available)
IEP/EI Reports/Other Reports (if applicable)
Income verification (Paystubs (2 consecutive within 6 months) or W2 Form)
Any completed enrollment forms from below (these can be completed during the appointment)

Please contact our Case Manager, Stephanie Creely, via email at ssteinys@cps.edu, to accept your School Assignment to begin the enrollment process.

Please note: All fillable forms must have an original parent/guardian signature; an electronic signature is not acceptable. By signing these documents, the parent/guardian certifies that they are authorized to complete the documents and that the information provided is true and accurate.
Information about vision services available to all students can be found on page 20, and the consent forms to enroll in these services are on pages 21 and 22. Consent must be completed before services are received. If you take your child to a private eye doctor, please ask those doctors to complete pages 23 and 24.
If any of the following pertains to your child, additional action is required:
• Chronic health condition: Consult with your child’s school nurse, who will provide forms to be completed by your health care provider.
• Food allergy: Ask your health care provider to complete the required forms and then submit the completed forms to your child’s school.
• Asthma: Ask your doctor to complete the required forms and then submit the completed forms to your child’s school.
