During this appointment, please bring the following:
Your child that will be attending our school.
Child’s Birth Certificate/Passport
2 Proofs of residence (ID/Drivers' License, Utility bill (gas, electric, water))
Immunizations/physical Forms (if available)
IEP/EI Reports/Other Reports (if applicable)
Income verification (Paystubs (2 consecutive within 6 months) or W2 Form)
Any completed enrollment forms from below (these can be completed during the appointment)

Please note: All fillable forms must have an original parent/guardian signature; an electronic signature is not acceptable. By signing these documents, the parent/guardian certifies that they are authorized to complete the documents and that the information provided is true and accurate.
Information about vision services available to all students can be found on page 17, and the consent forms to enroll in these services are on pages 19 and 20. Consent must be completed before services are received. If you take your child to a private dentist or eye doctor, please ask those doctors to complete pages 21 and 22.
If any of the following pertains to your child, additional action is required:
• Chronic health condition: Consult with your child’s school nurse, who will provide forms to be completed by your health care provider.
• Food allergy: Ask your health care provider to complete the required forms and then submit the completed forms to your child’s school.
• Asthma: Ask your doctor to complete the required forms and then submit the completed forms to your child’s school.
