A parent or legal guardian must enroll the student.
The parent or legal guardian must have 2 pieces of legal identification with current address
driver's license or state ID
current utility bill
insurance statement
bank statement
The child's birth certificate
2 consecutive paycheck stubs (required by the Office of Early Childhood)
Health Exams and Immunizations: Minimum Health Requirements
Important Note: A current physical is required for the beginning of preschool. Vision and dental exams are not required but encouraged.
