- A parent or legal guardian must enroll the student.
- The parent or legal guardian must have 2 pieces of legal identification with current address
driver's license or state ID
current utility bill
insurance statement
bank statement
paycheck stub
- Transfer documents from previous school (and ISBE form if outside of CPS)
- Health Exams and Immunizations: Minimum Health Requirements
Important Note: A current physical is required at the time of registration.

- If new to Chicago Public Schools:
the child's birth certificate is required
a transfer from the student's previous school is required
