- A parent or legal guardian must enroll the student. If the student is coming from a CPS preschool, please request a transfer from that school.
- The parent or legal guardian must have 2 pieces of legal identification with current address
driver's license or state ID
current utility bill
insurance statement
bank statement
1 months paycheck stub
- The child's birth certificate
- Health Exams and Immunizations: Minimum Health Requirements
Health Exam Form (required): Physical Examination requirements due upon enrollment. Physical Examination must be completed within one year prior to entry.
Dental Exam (required): Dental exam requirements due upon enrollment
Vision Exam (required): Vision exam requirement due upon enrollment
