*Our nurses provide medication organization and pillbox pre-fills based strictly on current, labeled prescription bottles and/or physician-provided medication lists. Member/Family is responsible for notifying nurse of any mid-month changes made by a physician.

"The Homecoming" Essentials (4–6 hour transition) | $400 +
*This package is perfect for same day surgeries where the first few hours home can be the most overwhelming.
Safe Escort- We meet you at the surgical center, manage pharmacy pick up, and provide a "hospital-to-bed" transition (our nurses do not personally transport patients, but can facilitate safe transport)
The "Nesting" Set up- We prepare your recovery space (pillows, water, chargers, cooling/heating pads, essential oils, etc.) so you don't have to move!
Clinical Baseline- Vital sign monitoring and a comprehensive assessment to ensure anesthesia is wearing off safely

*For patients who are discharging from hospital or rehab and are higher risk for readmission
Homecoming Audit- RN meets patient at home within 24 hours of discharge to ensure oxygen, medications and equipment are set up appropriately and per physician orders
Follow Up- Nurse coordinates and attends the initial post hospitalization follow up appointment
Caregiver Training- One on one training for spouse or caregiver on how to manage new medications, equipment, medical devices, etc

*This is a comprehensive, flat-fee package designed to take the burden off your family from the moment the decision is made until your loved one is tucked into their new bed
In home level of care assessment - Your RN completes a 90-minute assessment to determine true category of care needed (Assisted Living vs Skilled Nursing vs Memory Care.)
Records Review- Gather all pertinent patient records to ensure the facility receives a complete clinical picture
Top 3- Your nurse will compile the top 3 facilities based off of her clinical assessment and your budget
Guided Tours- Your nurse will accompany you on tours of selected facilities and provide a written pro/con list for each facility based on their most recent Missouri DHSS (Department of Health and Senior Services) inspection reports will be promptly provided
Admission Advocacy- Your nurse reviews your loved one's admission care plan during initial intake, to ensure all needs are being met, from medication to dietary needs
The First 48 Check In- Your nurse visits facility 48 hours after move in to ensure a smooth transition and address any "new environment" anxieties or medical discrepancies
