Notice of Privacy Practices For: Common Bond Nursing Concierge, LLC

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
I. Our Pledge Regarding Health Information
We understand that health information about you and your health is personal. We are committed to protecting your health information. We create a record of the care and services you receive to provide quality care and to comply with legal requirements. This notice applies to all of the records of your care generated by Common Bond Nursing Concierge, LLC.
II. How We May Use and Disclose Health Information
We may use and disclose your Protected Health Information (PHI) for the following purposes:
A. Treatment
We may use your health information to provide you with or coordinate your concierge nursing care services. This includes sharing your information with other healthcare providers involved in your care, such as your primary care physician or specialists, to ensure they have the necessary information for your treatment.
Example: We may share your nursing assessment and care plan with your cardiologist to coordinate your post-procedure follow-up care.
B. Payment
We may use and disclose your health information for activities that are necessary to bill and collect payment for the concierge services we provide to you.
Example: We may send a bill or invoice that includes information about your services to you or your designated personal representative for payment.
C. Healthcare Operations
We may use and disclose your health information for our business operations. These uses and disclosures are necessary to run the practice and ensure that all of our clients receive quality care.
Example: We may use your information to review the quality of our services, train staff, or conduct business planning.
D. Other Common Uses and Disclosures
We may also use and disclose your health information without your prior authorization for the following purposes:
As Required By Law: We will disclose health information when required to do so by federal, state, or local law.
Public Health Activities: We may disclose your health information to public health authorities authorized by law to collect or receive it for the purpose of preventing or controlling disease, injury, or disability.
Health Oversight Activities: We may disclose health information to a health oversight agency for activities authorized by law, such as audits, investigations, inspections, and licensing.
Lawsuits and Disputes: If you are involved in a lawsuit or a dispute, we may disclose health information in response to a court or administrative order.
Law Enforcement: We may release health information if asked to do so by a law enforcement official: in response to a court order, subpoena, warrant, summons or similar process; or to identify or locate a suspect, fugitive, material witness, or missing person.
Coroners, Medical Examiners, and Funeral Directors: We may release health information to a coroner or medical examiner. This may be necessary, for example, to identify a deceased person or determine the cause of death.
Military and Veterans: We may disclose your health information as required by military command authorities.
Workers’ Compensation: We may disclose health information for workers’ compensation claims or similar programs.
III. Your Rights Regarding Health Information
You have the following rights regarding health information we maintain about you:
Right to Inspect and Copy: You have the right to inspect and get a copy of the health information that may be used to make decisions about your care. We may charge a reasonable, cost-based fee for the costs of copying, mailing, or other supplies associated with your request.
Right to Amend: If you feel that health information we have about you is incorrect or incomplete, you may ask us to amend the information.
Right to an Accounting of Disclosures: You have the right to request an "accounting of disclosures," which is a list of the disclosures we have made of your health information, outside of those for treatment, payment, and healthcare operations.
Right to Request Restrictions: You have the right to request a restriction or limitation on the health information we use or disclose about you for treatment, payment, or health care operations. We are not required to agree to your request, but if we do, we will comply with that restriction unless the information is needed to provide emergency treatment.
Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. For example, you can ask that we only contact you at work or by mail.
Right to a Paper Copy of This Notice: You have the right to a paper copy of this notice. You may ask us to give you a copy of this notice at any time.
If you believe your privacy rights have been violated, you may file a complaint with me directly at info@commonbondcares.com or with the U.S. Department of Health and Human Services Office for Civil Rights. I will not retaliate against you for filing a complaint.
IV. Our Responsibilities
We are required by law to maintain the privacy and security of your PHI.
We must provide you with this Notice of our legal duties and privacy practices with respect to your PHI.
We are required to abide by the terms of the Notice currently in effect.
We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
V. Changes to This Notice
We reserve the right to change this notice at any time. We reserve the right to make the revised or changed notice effective for health information we already have about you as well as any information we receive in the future. The current notice will be posted at our office and on our website, if applicable, and you can request a copy at any time.VI. Complaints
If you believe your privacy rights have been violated, you can file a complaint with our Privacy Officer or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
We believe in transparency. Here is a simplified summary of our service agreement:
RN Scope of Care: As your Concierge Nurse, we provide expert nursing assessments, advocacy, and care coordination. However, we are not doctors. we do not diagnose medical conditions or prescribe medications. Our role is to bridge the gap between you and your primary medical team.
Emergency Protocol: Common Bond Nursing Concierge, LLC is NOT a first-responder service. If you are experiencing a life-threatening emergency, you must call 911 immediately. Our services are scheduled in advance and are not equipped for emergency intervention.
Cancellations & Fees: We value your time and ours. Cancellations made with less than 24 hours' notice are subject to a $50 fee. This allows us to keep our schedule open for other families in the Springfield area who may need urgent, non-emergency support.
As a concierge nursing practice, we operate under the highest clinical and ethical benchmarks. Our care is guided by the following pillars:
All services are performed in strict accordance with Missouri Revised Statutes Chapter 335. As a Registered Nurse, I maintain:
Active Licensure: Constant "Good Standing" status with the Missouri State Board of Nursing.
Scope of Practice: I provide nursing assessments, care coordination, and health education. I do not diagnose medical conditions or prescribe medications, but I work as a vital liaison between you and your prescribing physicians.
We follow the ANA Standards of Gerontological Nursing Practice. This means our care isn't just "medical"—it is Person-Centered.
Dignity-First Care: Every care plan is designed to honor the patient’s personal wishes and life history.
Advocacy: We act as your "clinical voice" in the healthcare system, ensuring specialists and facilities respect your goals of care.
Confidentiality: Your health data is protected under 45 CFR Parts 160-164. We use encrypted, HIPAA-compliant communication tools for all patient interactions.
Mandated Reporting: In compliance with RSMo § 210.115, we are committed to the safety of our seniors. We are legal mandated reporters for any suspected abuse, neglect, or exploitation.
