VIABILITY Pre-Employment Transition Services Program

Viability’s new Pre-Employment Transition program (Pre-ETS), through NYSED, is being offered to students between the ages of 14 and 21, who are potentially eligible to receive ACCES-VR services.

To apply for an eligibility intake meeting you need to complete the  following 3 forms:
1.  "Application/Release" form.
2.  "CONSENT COMMACK & VIABILITY to Exchange Information" form.
3.  "VIABILITY Consent" form.
You can access these forms below and fill them out online or you could print the forms then scan them once completed.

The 3 forms need to be signed and sent electronically to Pauline Shea at:

pshea@commack.k12.ny.us

You will then be contacted to attend an intake meeting to determine eligibility for services.
