ACCES-VR is an agency that can provide a variety of services to individual with different abilities. We recommend that students apply for ACCES-VR while in the school system as the appropriate supporting documentation can be provided as well as any necessary guidance through the process.
To apply for an eligibility intake meeting you need to complete both the application and both Release Forms. Please click on the following pdf links or you can click below and use the fillable forms through the ADOBE app.
The application and 2 release forms need to be signed and sent directly to ACCES-VR at the following address:
