Transcript Request - Eugene HS

You must turn in Transcript Request form, specifying where you want your final transcript sent, to Mrs. Adrian prior to graduation. Please make arrangements to do this by May 1, 2018. Remember, if you plan to play college sports, you must also fill out a final transcript request form for the NCAA/NAIA clearinghouse.

Transcripts request forms are downloadable from this site or can be picked up outside the counseling office or in the main high school office.
