THINGS TO DO AT THE BEGINNING OF THE YEAR:

Parents have to fill out the following forms at the beginning of the year:

1. The following forms need to be signed and returned to school:

• Code of conduct form (Necessary)
• Textbook accountability form (Necessary)
• Video/Photo release form (Necessary)
• Participation in PE permission (Necessary)
• Medication authorization (Only if needed)
• Asthma action plan (Only if needed)
• Authorization for self medication (only if needed)
• Diet order form (Only if needed)
