All Orchestra families must complete the following two forms before receiving an instrument.

1. NRMS Orchestra Contact Information Form - your submission will automatically upload to my Google Drive

2. CMS Musical Instrument Disclaimer Form below - Download the form, enter information, then send the scanned form with a parent/guardian signature to annef.grady@cms.k12.nc.us.
