Important School Health Information 

Please communicate directly with Carmel's School Nurse,  Tara Christensen (980-343-6705), for any health condition that impacts your child's performance at school, especially seizure disorders, sickle cell anemia, heart conditions, uncontrolled asthma; any condition that has the potential to turn into an emergency event.

Some things to consider:

· Please make sure your contact information is correct in PowerSchool.

· If your child has a chronic illness, please contact your provider and seek advice on your child’s return to school and any related recommendations for school staff.

· Please keep your child’s immunizations up to date. All children in the 7th grade must have the Tdap booster and the meningitis vaccine.

· If your child is in the 6th grade, as you take them for their routine checkups, please consider getting the necessary vaccines for the 7th grade.

· If your child is starting school in NC for the first time, you will need to present your child’s immunization record as well as current physical exam documentation.  We will accept the document even if from out of state, so long as, it was performed with in the last year and signed by the provider.

· If you have gotten a deficiency notice or phone call for needed immunizations or physicals, please follow through on this to avoid your child being excluded from school for noncompliance. Make your appointments now as providers/clinics are having to schedule out.

Health Related Forms and Documents

CMS requires specific documents, along with any type of medication, to be presented to the School Nurse before a child can self-carry or self-administer any type of medication.
