In order to represent Cleeve Park School and meet our school fixtures programme, students are often required to travel away for fixtures in the school mini bus. Mini buses will always be driven by qualified members of staff.
By signing the Parental Consent form you agree to the following:
I give consent for my child to attend extracurricular training sessions and fixtures
I give consent for my child to travel in the school mini buses
I give consent for my child’s photo to be taken during Cleeve Park sporting activities
I give consent for my child’s photo to be used to promote the PE Department’s activities
I understand that my child’s PE kit must be brought into school even if they are unable to take part in physical activity as they will still be given a role within the lesson
Cleeve Park School uses a biometric recognition system for the purposes of purchasing food in the canteen and operating some printers. The information from your child that we wish to use is referred to as ‘biometric information’ specifically a thumb/fingerprint. We are required to notify the parents of a child under 18 years old and obtain the written consent of at least one parent before being able to process a child’s biometric information for this purpose.
The written consent of at least one parent is required. However, consent given by one parent will be overridden if the other parent objects in writing to the use of their child’s biometric information. Similarly, if your child objects to this, Cleeve Park School cannot collect or use their biometric information for inclusion on the biometric system.
If you do not wish your child’s biometric information to be processed, we provide a PIN number for children who are not going to use the biometric system. You can withdraw consent you have previously given in writing at any time or request a withdrawal of consent form from the school office.
Please note that when your child leaves the school/college, or if they cease to use the biometric system, their biometric data will be securely deleted.

All visits (including residential trips) which take place during the holidays or a weekend
Adventure activities at any time
Off-site sporting fixtures

Curriculum based visits
The school will send you information about each trip or activity before it takes place.
You can, if you wish, tell the school that you do not want your child to take part in any particular school trip or activity, however, if the trip is directly related to the subject curriculum and takes place during the school day your child will be expected to participate and the school will provide you with information before the trip take places via letter or the school parent mail.
Please note that it is parent/carers’ responsibility to ensure that any changes in contact or medical details are notified to the school.
As the parent or carer, I grant permission for my son or daughter to have access to use the internet, school e-mail and other ICT facilities at Cleeve Park School.
I confirm that my child has read, understood and will follow the policy on IT use (available on the CPS website)
I accept that ultimately Cleeve Park School cannot be held responsible for the nature and content of materials accessed through the internet and mobile technologies, but I understand that the school will take every reasonable precaution to keep students safe and to prevent students from accessing inappropriate materials. These steps include using an educationally filtered service, restricted access e-mail, employing appropriate teaching practice and teaching e-safety skills to students.
I understand that my child will be provided with a username and password and it is my child’s responsibility to keep this information safe. I confirm that I will contact the school immediately if I suspect this password has been compromised.
I understand that Cleeve Park School can check my child’s computer files, take screen shots of inappropriate use/language, intercept and monitor emails and monitor the internet sites they visit. If the School have concerns about their e-safety or e-behaviour they will contact me.
I will support the school by promoting safe use of the Internet and digital technology and will inform the school if I have any concerns over my child’s e-safety.
I agree my child will use the internet and school technologies sensibly, safely and legally.
Emergency closure during the school day
If the school opens in the morning every effort will be made to keep the school open until the normal end of the school day at 3.00pm, whilst ensuring as far as possible that students will be able to travel home safely at the end of the day. In the event of severe weather or other unforeseen circumstances should it become necessary to close the school no student will be permitted to leave without prior written permission from parents/carers.
Parents/carers will be contacted by e-mail or text should the school be forced to close.
Parents/carers should ensure they complete the emergency closure consent below AND also complete the permission section in their child’s contact book.
In case of emergency closure of the school I give permission for my son/daughter to leave school.
Cleeve Park School may use photographs of my child or in video/audio material including them for the school website and social media platforms, newsletters and internal school use (i.e. displays).
I understand that images will only be used to support learning activities or in publicity that reasonably celebrates or promotes the work of the school, The Kemnal Academies Trust and for no other purpose.
We do not use students full names unless previously agreed with parents / carers.
