Band Supply Form - Needed for Marching Band items

Instrument Loan Agreement - Needed for anyone renting / using a school instrument

Parent Activity/Athletics Registration - All Marching Band students must have their parents fill out this form
***Band students do NOT need a physical. That is only necessary for Athletics***

Fee Waiver Form - If your student qualifies for Free / Reduced lunch,
please fill out the Fee Waiver form and list the band supplies needed. LPS will waive all required items.
