Parents/Guardians of students enrolled in the Christina School District must complete an Online Acknowledgment Form for the new school year to acknowledge that you have been made aware of the following forms and agree to complete and return them to the school, as necessary. Please print and complete the required forms and return to your school office.  If you are unable to print a form, or return them to the school office, please contact the office so we can work together to ensure all required forms are completed.

District 1:1 Technology Agreement Form

Photo and Video Opt-Out:  I understand that CSD allows photographs and video footage for the purposes stated on the Student Photo/Video Opt-Out Form. I acknowledge that I have received this form and will complete and return to my child's school if I DO NOT want your son/daughter’s photographs or videos to be used.

Emergency Information Card:  I understand that a NEW emergency card must be completed each year. I acknowledge that I have received this form and will complete and return to my child’s school.

Title I Compact: I make a promise to do my part by acknowledging my the parent portion of the Title I Compact.

Student Manual:  I acknowledge that I have reviewed and discussed the Christina School District Student Manual with my child.

Hand Sanitizer: Available for Personal Use Notice

Meal Benefits Form: I understand that annually families are offered the opportunity to apply for federal assistance which may result in free or reduced priced meals. In order to apply, a NEW meal benefit form must be completed each year. I acknowledge that I have received this form and will complete and return to my child's school.
