The Evaluation of Faculty Teaching Performance under PC-TAL-WI.01 aims to determine the teaching performance and improve the delivery of instruction of the faculty member. This activity is part of the Work Instructions of the Program Chairperson which is conducted twice in an Academic Year and covers the evaluation and observations of faculty in a classroom setting.
Prior to the classroom observation, the program chairperson prepares the Schedule of Observation Form once per semester and forwards it to the Office of the Dean for approval. The approved communication is being disseminated to all concerned faculty members and has it acknowledged by the latter. In the conduct of the classroom observation based on the approved schedule, the program chairperson sits- in during the class discussion and checks on the course syllabus and class records of the faculty being observed. The faculty is being evaluated using the Classroom Observation Form with the evaluation criteria for Lesson (25%), Content (25%), Teaching Methods (20%), Classroom Management (10%), and Questioning Skills (20%). The faculty is rated on a scale of 1 (Poor) to 5 (Outstanding). The mean scores are determined per evaluation criteria and summed up for the overall Classroom Observation Rating (COR). Also, part of the evaluation process is the Strengths Observed and the recommended Areas for Improvement by the program chairperson to the faculty observed. At the end of the classroom observation, the program chairperson discusses the result of the evaluation to the faculty concerned and has the form signed by the faculty as conformity to the result. The accomplished Classroom Observation Forms are compiled by the program chairperson and forwards it together with the summary to the Office of the Dean for filing.
The review of examination questionnaires aims to ensure the quality of content, format and coverage of the examination questionnaire given to the students at the end of every term. This particular work instruction of the program chairperson is conducted before the scheduled term
examination.
The program chairperson collects the examination questionnaire submitted by the faculty at least 3 days before the scheduled term examination. The faculty signs the logbook or monitoring form provided by the program chairperson to acknowledge submission of exam questionnaire. The program chairperson reviews the exam questionnaire against the table of specification to ensure questionnaire contains the prescribed topics in the course syllabus, test items require higher order thinking skills, and test formats/types are appropriate for the course and program. Then, the program chairperson returns the reviewed questionnaire to the respective faculty with acknowledgment of examination review form. For any comments or corrections, exam questionnaire will be returned to the respective faculty for revisions. Lastly, the program chairperson submits the acknowledged examination review form and the corrected examination questionnaire to the Dean for approval and reproduction.
The PC-TAL-WI.03 aims to establish the process in the screening of new students and transferees. This activity is part the Work Instructions of the Program Chairperson which is conducted at the beginning of the Academic Year and covers students in the undergraduate programs.
One of the functions of the program chairpersons is to screen incoming students and transferees from other schools. It is the responsibility of the chairperson to check their credentials such as
photocopies of Form 138 (Report Card), Transcript of Records (for transferees), medical certificate and 1 pc. 2x2 ID picture. When the students pass the initial screening, the chair then conducts interviews and issues the admission slip for the qualified entrants. As for the transfers, the chair fills up the course crediting form and identifies the course to be credited. Then recommends the credited courses for approval of the Dean, prepares the list of qualified applicants for approval of the Dean and issues the admission form for the qualified students.
Evaluation of students’ academic progress is established to process the admission and evaluation of students’ undergraduate program. This particular work instruction of the program chairperson is conducted one month before the start of enrollment for the semester to ensure all courses to be taken/enrolled by the students are in accordance with the curriculum.
The program chairperson monitors and evaluates the grades of the students based on the prospectus, rating report issued by the registrar’s office and/or retention policy, if applicable. Then, issues the students loading slip containing the courses to be taken/enrolled by the students for the upcoming semester. Specifically, the students loading slip contains the course code, course title, number of units and the section of the students. The registrar’s copy of the students’ loading slip is forwarded to the registrar’s office, while the chairperson’s copy is kept on file. In addition, any request of subject/s to be dropped or added by the students would be reviewed by the program chairperson before submission to the registrar’s office.
The purpose of this work procedure is to establish the process of review, revision and endorsement of the curriculum. The Dean together with the Program Chairpersons ensure that the current curriculum is in consonance with the requirements of Policies, Standards and Guidelines of the programs.
The curriculum review commences with the Dean reviewing the proposed curriculum submitted by the Program Chairperson or the Coordinator. After careful review of the proposed curriculum, the Dean then organizes for a stakeholders’ consultative meeting where members from the other HEIs are invited, so too with the alumni, industry experts, and students. Their inputs are considered in the final preparation of the proposed curriculum. The proposed curriculum is being presented to the Local Academic Council for comments and suggestions before it is presented to the Academic Council. Once the proposed curriculum is endorsed by the Academic Council, it is being endorsed by the ACAD to the members of the Board of Trustees for approval. Once the curriculum is approved, the Deans together with the Chairpersons, implement the approved curriculum.
The PC-TAL-WI.06 aims to establish the process in assigning teaching load to the faculty members of the undergraduate programs. This function is part of the Work Instructions of the Program Chairperson which covers the teaching load assignment. It is being done at the beginning of each semester within the Academic Year.
The program chairperson’s responsibilities are to prepare the course offerings for each term whether it is on a regular semester or during summer. After all the courses to be offered had been identified, the chair identifies the faculty who will teach the course. He or she then fills out the faculty teaching load form and submits this to the Dean for review. After review of the Dean and if there are revisions, the chair incorporates the correction of the Dean in the final load form

The first phase of the review starts by determining the members of CRRS or the Committee on Review and Revision of the Syllabus. The program or the area chair serves as the chair of the committee. He or she shall determine the faculty members to be part of the CRRS. The CRRS chair and the members look into the details of the syllabus and evaluate whether the syllabus has followed the guidelines written in the evaluation form. The usual recommendation on this phase is “for revision”. The second phase of the syllabus review is participated by the same people present during the first phase: the proponent, the CRRS chair, and the CRRS members. The CRRS ensures that the recommendations from the first phase review are incorporated on the revised syllabus. When the CRRS chair and members approve the revised syllabus, they attach their signatures on the syllabus and the course outline prepared by the proponent. After that, the proponent will seek approval for the use of the course syllabus from the degree-granting dean by affixing his or her signature. Lastly, the proponent submits the documents to the program chair and the program chair then endorses the revised syllabus and the course outline to the CIMD director/coordinator.
