Finalize the Members Information Form - download and Data Form - download and complete all the details, especially those of the officers, since these will be used in processing your registration.
The officers should only be those who attended the seminar and are included in the PRS Certificate of Attendance. All ELECTED OFFICIALS are not allowed.
Create an email address or use a personal email address that will be used for the registration.
Secure a Surety Bond. You may obtain it from insurance companies. The Treasurer should be the one bonded. Suggested providers where you may get a cheaper surety bond:
a. CLIMS – their office is located behind Balangasan Elementary School, at the Zamsureco Coop Building, Pagadian City. Only one person may process the form as long as it is signed by the Treasurer and two co-signers, preferably the Chairperson and Manager. b. 1-CISP – you may contact them directly for instructions since they also allow GCash transactions. Contact person: Ms. Cecile B. Waga – 09179846502. Or you may contact their branch/satellite in your area.
Start collecting the agreed share capital contribution (₱1,000 – depending on the agreed amount) and membership fee (₱100 – depending on the agreed amount). The capital contribution of members who are willing to join the cooperative may be collected gradually, but it is preferred that the total amount be collected before registration. Please explain carefully, especially to the Treasurer, that he/she will sign and notarize the Treasurer’s Affidavit stating that the total share capital and membership fees from the members have already been collected.
Follow up the PRS Certificate from CDA so it can be emailed or claimed from the office.

5.AREA OF OPERATION
6.CAPITALIZATION – not less than 15 thousand pesos
7.PAR VALUE OF EVERY SHARE – not less than 100 but not more than 1000 pesos
8.INTERIM OFFICERS and STAFF – note on prohibitions and relationships
9.DATE OF REGULAR GENERAL ASSEMBLY (maybe fixed or within 90 days from the end of fiscal/calendar year)
10.Email address – Official/personal
11.Compliant Officer – will accomplished the registration of the proposed cooperative in the e-coopris.
