Print this Personal Track Record as often as you need to keep personal records of your hours. CCS does not need to collect.
Fill out the Verification Form (below) and click Submit Form, your submission will be sent to your Faculty Advisor as well as the employer, mentor, coach, or teacher for the adult (not-parent, please) verification.
Beginning July 1 prior to a student’s ninth grade year at CCS and executed over their high school years at CCS, students need to meet the Department of Workforce Development’s five Employability Skills Benchmarks listed below. CCS will track these skills as students submit record of their evidence-based experiences

CCS students must complete a minimum of 20 hours per school year with a total of *80 hours for this requirement beginning with the class of 2023.
Though it is encouraged that the Employability Skills experience involve an activity, project, or organization related to the CCS student’s postsecondary interests, it is not required that the experience is aligned with those interests.
These experiences should be sustained over a period of time and not be episodic.
CCS students should collect evidence of progress toward meeting specific service goals and learning outcomes from multiple sources throughout their experience.
Each semester between December 1-15 and again between May 1-15, or the first Monday thereafter, CCS students will be required to submit their Work Product Proof to show their Employability Skills experience for the current semester.
If a student participates in Work Based Learning, a copy of the student's Work Permit (Intent to Employ must also be submitted.
All experiences require students to create a work-product (evidence) that demonstrates their development of employability skills such as a presentation, portfolio, or letter of employment verification.
NOTE:
For new students, CCS currently has discretion for making judgment calls regarding work in the Employability Skills experience and in pathways with particularly local flavors. CCS must make progress on pathways part of the conversation with new students; similarly, CCS should discuss questions of partial completion with the sending school.

Project-Based Learning includes working towards completing a carefully planned idea that allows students to gain knowledge and skills by working for an extended period of time to investigate and respond to an authentic, engaging and complex question, problem, or challenge. Students often make their project work public by explaining, displaying and/or presenting it to people beyond the classroom. An example of PBL would include an independent study program in which students directly apply what they have learned in school to a project that may range from a career interest to a hobby to something they would like to learn more about. The project is intended to be a student-led learning experience with set guidelines and milestones along the way.
CCS students must submit at least one of the following as evidence of meeting PBL hours. To submit the following, please email the document to your Faculty Advisor.
a. Presentation: notes, PowerPoint, or proof of a public presentation of the project completed.
b. Letter of Recommendation: General letter of recommendation (letter could be from a mentor, teacher, coach, etc.).
*c. Project Verification Form: Please print a copy of a letter and form (below) to submit to an authority figure to fill out and return to you to email to your Faculty Advisor.

Service-Based Learning (SBL) involves an activity that integrates meaningful service to enrich and apply academic knowledge, teach civic and personal responsibility (and other employability skills), and strengthen communities. SBL should be solution-oriented for CCS students. These experiences can be as diverse as the communities we serve. Meaningful service engages students by allowing them to plan, organize, implement, participate, and evaluate their experience.
SBL hours are eligible to be recorded on transcripts.
CCS students must submit at least one of the following as evidence of meeting SBL hours. To submit the following, please email the document to your Faculty Advisor.
a. Presentation: notes, PowerPoint, or proof of a public presentation of the service completed.
b. Letter of Recommendation: General letter of recommendation (letter could be from a mentor, teacher, coach, etc.).

Work-Based Learning (WBL) includes activities that occur in workplaces and involve an employer assigning a student with meaningful job tasks to develop his or her skills, knowledge, and readiness for work. For CCS students, an employer partners with CCS to provide a complete assessment of student experience in the WBL program. This will include participants’ skills and learning outcomes as they are officially recognized and documented.
WBL hours must accompany an Intent to Employ.
CCS students must submit at least one of the following as evidence of meeting WBL hours. To submit the following, please email the document to your Faculty Advisor.
a. Resume: Detailed description of activities in the appropriate format.
b. Letter of Recommendation: General letter of recommendation (letter could be from a mentor, teacher, coach, etc.).

WBL includes activities that occur in workplaces and involve an employer assigning a student meaningful job tasks to develop his or her skills, knowledge, and readiness for work. It supports entry or advancement in any particular career field and can serve as the culminating course or event in a student’s chosen career pathway. Through WBL, students have the opportunity to apply the concepts, skills, and dispositions learned in previous coursework in real world business or industry settings.
