Emergency Information
Emergency contact information should be updated annually. Please complete the attached School Health Services Student Information sheet/card and return it to the school as soon as possible. This information will enable us to contact your whenever there is need. If any information changes during the school year, contact the school immediately.
Student Illness/Injury
Only minor treatments will be provided in the clinic. Major concerns can be evaluated, but will need to be followed-up by your family physician. If your child frequently complains of a problem, you will be notified by phone or letter. Sick student who are contagious or have a fever (>100.0) must not be sent to school. When a student becomes ill at school, the parent must arrange for the student to be taken home. If your child has an emergency, all efforts will be made to contact you immediately. It is extremely important that the school and clinic have an up-to-date operating phone number where your can be reached during school hours.
Prescription/Non-Prescription Medications
If a medication must be taken at school, an Authorization to Give Medication at School form must be completed. If the medicine is to be given greater than ten (10) days, the prescribing physician must also sign this form. If the dosage changes, a new authorization form must be completed. Any medication brought to school by the student must be taken directly upon arrival to the office or clinic, along with the authorization form. All medication should be brought to school in the original container or wrapping. Any prescription medication must be in a current prescription bottle.
Health Care Plans and Emergency Medications:
The parent or guardian of a student with a chronic medical condition is required to provide the school with a Health Care Plan and prescribed emergency medications as soon as possible and have it updated yearly. Chronic medical conditions include but are not limited to asthma, diabetes, and allergic reactions. As a safety precaution, students with chronic medical conditions that attend summer school should provide a copy of the Health Care Plan to the summer school principal. Repeated failure of a parent or guardian to provide the school with a Health Care Plan and/or necessary medications may be considered neglectful and such neglect may be reported to the proper authorities.
