If you are new to Georgia, your out-of-state immunization records will need to be transferred to the Georgia Form 3231. This can be done at the health department, DDEAMC, or your child's pediatrician.
If your child has a medical condition that may require medication or emergency intervention at school, please let the nurse know right away. Click on the HEALTH CARE PLANS tab under Helpful Links for more information and the required forms.
Immunization requirements vary from state to state, so your child may require additional vaccines in order to be compliant with Georgia law for public school attendance if transferring from another state.

A valid Form 3231 Certificate of Immunization is required for attendance at all Georgia public schools. This form can be obtained from the local health department or your child's pediatrician. The expiration date of the Form 3231 is based on the child's birthday rather than the school year. It is the parents responsibility to keep their child's vaccines up-to-date. Please bring in your child’s current Form 3231 or ask the school nurse to pull records from the online vaccine registry (GRITS) after a vaccine appointment. Click here for more School Immunization Information. DPH Form 2208 is accepted in lieu of Form 3231 for applicable students and does not expire. Medical exemptions must be documented by a physician directly on the Form 3231 in the far-right column and must be updated annually. This must be turned in within 30 days of the student's first day of attendance. Students without a valid Certificate of Immunization or exemption may not be allowed to attend school per Georgia Law.
When entering a Georgia school for the first time or having been out of a Georgia school for 1 full school year, a 4 point health screening is required: Ear, eye, dental and nutrition assessment. This health assessment must have been conducted within one year prior to the time that the child is admitted for the first time to a Georgia public school. This certificate can be obtained by visiting your child's pediatrician, Eisenhower Army Medical Center, or the local public health department. This must be turned in within 90 days of the student's first day of attendance.
