ADMISSION PROCEDURES FOR NEW STUDENTS AND TRANSFEREES FOR SENIOR HIGH SCHOOL

IN PERSON ENROLLMENT PROCEDURE
For incoming Grade 11 student, listed below are the grade requirement for each strand:
STEM -85% average for SCIENCE, MATH, and ENGLISH
ABM - at least 85% for MATH and ENGLISH
HUMSS/GAS - 80% average for all subjects
STEP 1. CONFIRM YOUR STRAND PLACEMENT FROM THE REGISTRAR
STEP 2. FILL UP THE ENROLLMENT FORM AND SUBMIT THIS TO THE ADMISSIONS OFFICER ALONG WITH THE FOLLOWING DOCUMENTS:
Initial Requirements that should be submitted upon enrollment:
a. F138 / SF 9 (Original and 2 pcs Photocopy of Report Card -G10 or G11)
b. Certificate of Completion (Photocopy)
c. Certificate of Good Moral Character (Original)
d. NCAE Result (If available)
e. ESC/QVR Certificate (Original) (if enrolee is not a continuing CI Junior High School student)
f.Philippine Statistics Authority ( PSA )Birth Certificate (2 pcs. Photocopy)
g. 1 pc. 2x2 Colored ID Picture
If the PSA birth certificate is not immediately available submit the scanned/photocopied birth certificate issued by the Municipal Civil Registrar.
FOR CONTINUING STUDENTS, listed below are the following requirements upon enrollment:
a. F138 / SF 9 or Report Card (Original)
b. 1 pc. 2x2 Colored ID Picture
FOR FOREIGN STUDENTS, listed below are the following requirements upon enrollment:
a. Passport (Scanned Clear copy)
b. ACR I-card (Scanned Clear copy)
c. Transcript of Records
d. Basic Requirements (same as above – NEW / TRANSFEREES)
STEP 3. ASSESSMENT OF FEES
After submitting the documents to the ADMISSION'S OFFICE, proceed to the REGISTRAR to know your placement and fees. or continuing students with accounts payable settle your account with the Finance Officer before proceeding to the Cashier.
For scholarship applicants, kindly check the webpage under "SCHOLARSHIPS OFFERED".
STEP 4. PAYMENT
Once your strand placement and assessment of fees are confirmed, proceed to our CASHIER to pay your enrollment fees.
STEP 5. HEALTH PROFILING
For health profiling, refer to School Clinic Health Interview schedule in the CI Facebook Page
STEP 6. SCHOOL ABILITY TESTING
For school ability testing, refer to Student Wellness School Ability Test schedule in the CI Facebook Page
STEP 7. BOOKSTORE
P.E. uniforms and notebooks are available at our Bookstore while school uniform will be scheduled.
* Note: You are considered officially enrolled once you have paid the initial school fees and submitted the original copies of the required documents above. If a student has a remaining fund balance, kindly see the Finance Officer to settle your account.
ON-LINE ENROLLMENT PROCEDURE FOR NEW STUDENTS, TRANSFEREES & CONTINUING STUDENTS
STEP 1. FILL UP THE MODIFIED LEARNER ENROLLMENT AND SURVEY FORM & ATTACH THE REQUIREMENTS NEEDED
Forms to be accomplished:

Along with the Enrolment Survey Form Submit the following scanned documents, but original copies must be submitted by August 30 or earlier
a. Report Card
b. Philippine Statistics Authority (PSA) Birth Certificate
c. Certificate of Good Moral Character
d. 2x2 picture
e. Vaccination Card (photocopy only)
If the PSA birth certificate is not immediately available submit the scanned/photocopied birth certificate issued by the Municipal Civil Registrar.
Please note that for CONTINUING STUDENTS, you are only required to submit your report card, 2 x 2 picture, and photocopy of your vaccination card.
Step 2. CONFIRMATION OF STRAND PLACEMENT AND FEE ASSESSMENT
The Registrar will confirm your strand placement and provide your fee assessment through your contact address.
REMINDER: For continuing students with remaining balances / accounts payable for S.Y 2023 - 2024, settle your fees and get clearance from our Finance Head, Ms. Mercy Alvaran.
STEP 3. PAYMENT
Once you have received an email or confirmation from CI Admissions section regarding your unofficial Enrollment Assessment Form (EAF) you may pay through our payment partners or directly at the school cashier. Pay the first installment of school fees based on your EAF to the Cashier at the Administration Office or to the following CI depository banks.

FOR PROOF OF PAYMENT
Send a scanned copy of the proof of payment to cashier@caviteinstitute.edu.ph if paid through payment partners. Wait for 5 working days to confirm/verify your payment.

Payment– Last Name_First Name_Middle Initial_ Grade Level_Date deposited
(Example: Payment – Dela Cruz_Juan_M_Grade 7_May 31,2022)
