REQUIRED DOCUMENT LIST FOR 16-21 YEAR OLD PARTICIPANTS:

You can scan or take a picture of your documents, then upload to the DYCD participant website. Click on each requirement below to see a visual example.

1. Signed Social Security Card 

2. Proof of Identity (non-expired)

3. Proof of Age

4. Proof of address (school transcript preferred)

5. Proof of school/Employment Authorization

6. Proof of disability (ONLY if applicable)

7. Resume (If you don't have one, we will provide a workshop)

8. Signed W-4 

9. Signed PES survey

10. Signed 1-9 form

The following are additional required documents based on specific ages:

16-17 years old ONLY:

11. Signed Working Paper Card

Males 18 years old and up:

12. Signed Selective Service Receipt or screenshot
