1. Signed Social Security Card 

2. Proof of Identity (non-expired)

3. Proof of Age

4. Proof of address (Dated within the last 6 month, must show full address)

5. Proof of disability (ONLY if applicable)

6. Signed PES survey

7. Signed 1-9 form
