• To make valuable information readily available to parents/carers and family members

• Provide additional information to those in early stages of the Autism Assessment Journey

There are a number of transitions that pupils will have to go through during their time at school. Your child’s transfer from Primary to Post Primary school can be an anxious time for the family. Transitions can provide many challenges for our children with ASD and therefore it is important that we plan effectively to ensure that these transitions happen as seamlessly

as possible.

This pack aims to support parents and pupils moving from Primary to Post Primary by providing a range of strategies to address some of the challenges that our young people may experience. In addition to the pupil orientated resources, this pack contains an Individual Pupil Profile which can be completed by you and shared with the Post Primary school.
