Fee Waiver/Payment Plan Application

If you have applied for Free or Reduced Meals, you must also complete and turn in the Waiver of School Fees form. This form serves two purposes:

Option 1: Waiver of School Fees - For households who are approved for Free or Reduced meals, this application waives school fees. (Fill out the top portion, Fill out Part 1, skip Part 2, sign and date).

Fees not waivable: (Including but not limited to) Music Classes, Driver's Ed., Parking Stickers, Club and Athletic fees.

Option 2: Payment Plan - For households who cannot pay the full fee amount during Registration/Book Pick Up, you may request a short-term loan (Payment Plan).  Please read Part 2 of the application. (Fill out the top portion, skip Part 1, fill out Part 2, sign and date.)

Please bring your completed application when you attend Registration/Book Pick Up and turn in to the Free/ Reduced Meals Station.  Paper copies will be available if you are not able to print a copy. During the school year you may turn your application in to the appropriate administrator:    Al Daniel at Belleville East, Jason Karstens at Belleville West.
