Posted here are any important forms that are handed out to cadets in class at the beginning of the year. If a cadet needs or has lost a form, they should print out the attachment.

Parental Consent Form

Allows cadets to participate in fitness program, provides for cadet photo release, and allows upperclass cadets to input community service hours into AFJROTC database.

25-26 POV Transportation Consent

Transportation Permission Slip

Allows cadets to travel in vehicle with JROTC instructor when a school bus is not used.
