Any employee eligible for insurance (>.5FTE), and any spouse on the district health insurance may participate
The quiz is not mandatory in order to participate in the other incentives

Participation requirements
Any employee eligible or the district health insurance and/or souse on the district medical must submit the preventive exam affidavit (Signed by their doctor to the Benefits & Wellness Coordinator no later than April 30th, 2026.
Annual Incentive Break Down
1st Year - $250 (One Time)
Consecutive Years - $175
Every 5th Consecutive Year - $275
Non - Consecutive Years of Participation - $150
Payables
If sumbitted between May 1, 2025 and November 30, 2025, the incentive will be paid on the December 2025 payroll
If submitted between December 1, 2025 and April 30, 2026, the incentive will be paid on the June 2026 payroll
No forms will be accepted after the April 30, 2026
$100 - INCENTIVE LEVEL #2 - POINT MODEL PARTICIPATION
Any employee and/or spouse on the District medical plan is eligible if they have completed the following:
Complete Incentive Leave #1 - Preventive Wellness Exam Form
Earn $ through any combination of activites
Payable on the June 2026 payroll
