Physicals:

State of CT Physical form 

Immunization Requirements 2024-25

Per the state of Connecticut and the Brookfield Public Schools a physical must be done during the 6th grade year. The physical must be done by an MD, DO, APRN, or PA. 

• SIXTH GRADE PHYSICAL- MUST be done within the 15 months prior to the first day of 7th grade.  A student will not be allowed to return to school until the physical and updated immunizations are done and approved by the school nurse prior to entry. 

• NEW STUDENT TO BROOKFIELD  PUBLIC SCHOOLS-  A complete physical assessment must be done 12 months prior to the first day of school entry.  If arriving from outside of the United States, a complete physical must be done in the United States (and meet the CT state requirements) within the 12 months prior to the first day of school entry.  A physical assessment done on a USA military base outside of the USA will be accepted.

Sports:

All students participating in WMS interscholastic sports MUST have a current physical on file in the nurse’s office.

• A student  WILL NOT BE ALLOWED to try out or participate in a WMS interscholastic  sport until the physical is approved by the school nurse. 

• Per the CIAC, a physical is considered current for 1 year with a 1 month grace period for the student athlete to receive a new physical.  For the 2020-21 school year only,  the CIAC has extended physicals to 15 months- click here for details. 

• The WMS student athlete and a parent/guardian MUST also complete and electronically sign the following in the Parent Portal in Power School:

  1.  Concussion and Sudden Cardiac Arrest forms

  2. Check off all sports the student may be interested in for the current school year.
