Physicals:
Immunization Requirements 2024-25
Per the state of Connecticut and the Brookfield Public Schools a physical must be done during the 6th grade year. The physical must be done by an MD, DO, APRN, or PA.
SIXTH GRADE PHYSICAL- MUST be done within the 15 months prior to the first day of 7th grade. A student will not be allowed to return to school until the physical and updated immunizations are done and approved by the school nurse prior to entry.
NEW STUDENT TO BROOKFIELD PUBLIC SCHOOLS- A complete physical assessment must be done 12 months prior to the first day of school entry. If arriving from outside of the United States, a complete physical must be done in the United States (and meet the CT state requirements) within the 12 months prior to the first day of school entry. A physical assessment done on a USA military base outside of the USA will be accepted.
Sports:
All students participating in WMS interscholastic sports MUST have a current physical on file in the nurse’s office.
A student WILL NOT BE ALLOWED to try out or participate in a WMS interscholastic sport until the physical is approved by the school nurse.
Per the CIAC, a physical is considered current for 1 year with a 1 month grace period for the student athlete to receive a new physical. For the 2020-21 school year only, the CIAC has extended physicals to 15 months- click here for details.
The WMS student athlete and a parent/guardian MUST also complete and electronically sign the following in the Parent Portal in Power School:
Concussion and Sudden Cardiac Arrest forms
Check off all sports the student may be interested in for the current school year.
