Preschool Influenza Vaccine Requirement:

All preschool students are required to have at least one dose of influenza vaccine each year given between August 1st and December 31st.  Provide documentation of vaccination to the school nurse. Preschool flu vaccine FAQs 


Physicals and Vaccines:

A complete physical by a MD, DO, PA or APRN  and vaccine record done within 12 months prior to the entry date of preschool and of kindergarten are required, as well as for any new student entering the district. When entering 1st grade from another school district, the kindergarten physical will be used providing that the physical meets all the State of CT kindergarten requirements. If arriving from outside the United States, a physical must be done in the Unites States within one year prior to the date of school entry, and meet CT state requirements. Physicals done outside the United States on a US military base will be accepted. Click on these links for more information:

Vision and Hearing Screenings

Per state guidelines, students in grade 3 and 4 are required to have vision and hearing screenings done . To exclude your child from these screenings please submit a copy of previous screening done within 12 months. Nursing will notify you of any concerns regarding your child's screening results.
