Physicals:
A completed physical, State of CT Health Assessment Record, is required during the 9th grade year.
Sports at BHS:
All students participating in sports or practices, must have a current physical on file in the clinic. Physicals are current for 13 months. The health information page on PowerSchool must also be completed and signed by a parent/guardian.
Medications/Epipens/Inhalers:
All forms must be returned to the clinic before school starts. Students are ineligible for any field trips without a doctor's form on file in the clinic.
All in school medications, both prescription and OTC require a medication form signed by a doctor. Inhalers and Epi-pens are the only medication allowed to be self-carry and require a doctor and parent signature on the form allowing the student to self-carry.
Overnight field trips: All medications, both prescription and non-prescription, needed on an overnight field trip require a medication form to be filled out and signed by a doctor. Medication and form must be given to the clinic one week prior to the start of the trip by a parent/guardian.

Concussion and Sports injuries:
All concussions/sports injuries require a medical doctor signed note brought to the clinic the next school day. Progress notes on injury/concussion should be brought to the clinic the day following your appointment. If a concussion is diagnosed, the concussion protocol will be followed and a medical doctor’s note is required to return to all academic work and sports. All sports injuries require a doctor’s note to “return to play” for both sports and PE.
