Transition of Students with Disabilities to Postsecondary Education: A Guide for High School Educators (US Department of Education):  The purpose of this guide is to provide high school educators with answers to questions students with disabilities may have as they get ready to move to the postsecondary education environment. Addresses the admissions process, documentation, IEPs, 504s, obtaining services in college, and self-advocacy.

Wisconsin Division of Vocational Rehabilitation:  DVR helps people with disabilities find a job, keep a job, or get a better job.


General Resources

Understood.org: Provide resources and support so people who learn and think differently can thrive — in school, at work, and throughout life.

Wisconsin Bureau of Aging and Disability Resources:  Includes services for people in Wisconsin who are blind/visually impaired, deaf, Deaf-Blind or hard of hearing, and those living with physical disabilities.

Wisconsin DPI Transition Planning for Students with Disabilities:  Transition planning is helping students with disabilities and their families think about their life after high school. The process helps students identify long-range goals, designing the high school experience to ensure that students gain the skills and connections they need to achieve these goals after high school. Transition includes both annual and cyclical federal data collections and monitoring.

Wisconsin Statewide Parent Educator Initiative: Includes the Wisconsin Transition Resource Guide which covers practical tips, activities, essential services, and more to help high school students successfully transition into adulthood.
