Health History Form
This form is required to be filled out by the parent for each season. ONLY valid for 90 days from dated signature, so please date appropriately.
For example, if your child played soccer in the fall and submitted both the physical and the history forms, then they would need to complete a history form for the winter season and/or spring season.

Physical Examination Form
Your child will need a physical in order to participate. Please have your pediatrician complete the form attached prior to the season deadline.
Send in your child's physical form to the BCMS School Nurse.

The 5 Releases forms are in your parent Genesis. This will cover all health and safety information for the 2023-24 school year.
Required Guidelines
State law requires that a current, cleared PHYSICAL EXAMINATION form be on file in the health office for a student wishing to try-out/participate in a specific interscholastic sport. The physical examination shall be conducted within 365 days prior to the first practice session (try-out session). It is highly recommended that your primary care physician complete the Physical Examination, however, the student's parent/guardian may choose either the child's own private physician or the school physician to perform the physical examination.
