New Health Charlestown, from right around the corner on 15 Tufts Street, is a partner of Charlestown High School. Our School Based Health Center can provide such services as assessment and treatment for many acute or chronic ailments, behavioral health support, sports physicals, adolescent health, nutrition, immunizations, laboratory testing, health education, specialty referrals, and more. Whether your student is a patient of New Health or seen for primary care somewhere else, we are happy to offer services where they spend most of their time. We will communicate with you and their primary care office to ensure the best care possible.
If you would like your student to be able to access care with our clinic please complete this consent form. They can return it to us directly, or in our consent form box in the nurses area. Please feel free to give us a call with any questions. We look forward to being another supportive team for you among many here at Charlestown High School!
