SYMPTOMS
My child has symptoms that might be COVID (for children and youth, the symptoms are fever, new cough, vomiting, diarrhea)
Someone in the child's household has symptoms that might be COVID (for adults, the symptoms are fever, chills, new cough, nausea or vomiting, diarrhea, shortness of breath, muscle or body aches, fatigue, congestion, sore throat, headache, loss of taste or smell)
My child spent time with someone who had symptoms that might be COVID (for adults, the symptoms are fever, chills, new cough, nausea or vomiting, diarrhea, shortness of breath, muscle or body aches, fatigue, congestion, sore throat, headache, loss of taste or smell)
CLOSE CONTACTS (EXPOSURES)
My child was a close-contact (shared indoor airspace for 15 minutes or more) with a person who had tested positive for COVID outside of school
My child was determined to be a close-contact with a person who had tested positive for COVID during school or a school activity
