Pre-School Assessment Process

In some cases, a student may become eligible for special education supports through a public school district beginning on their third birthday. In order to determine whether a student is eligible for services or programs beginning at age three, the district must first conduct an assessment.

This process may also be used if your child is a an Early Start Regional Center Client and has been referred to the District for Services.

Please complete and sign both pages of the enrollment form, then bring those pages, along with proof of residence within Bonita Unified School District, to the Office of Student Services at 115 W Allen Ave, San Dimas, CA 91773. Once all documentation has been verified, the assessment process can be initiated.
