In this section you will need the following information to complete this form:

1. Download and print out the correct health forms:

   1. Bogota High School health forms (Grades 7-12)

   2. Bixby and Steen Elementary health forms (Grades K-6)

   3. Preschool health forms (children ages 3-4) 

2. Obtain an updated copy of the student's immunization form 

3. Scan or photograph the files
