In this section you will need the following information to complete this form:

For this form to upload properly, you need a gmail account.  Gmail accounts are free to create. (www.gmail.com) If you do not want a gmail account, you will need to print, complete, scan, and email the files to balmanzar@bogotaboe.com.  For more information or questions, call the Bogota Board of Education at 201-441-4800 x1009.

For homeowners (2 requirements)

• 1. Homeowner Requirement 1: If you are homeowner upload a copy of one of the following documents: Deed, Mortgage statement or current tax bill.

  2. Homeowner Requirement 2: Please provide 2 bills such as gas, cable, electric, credit card or bank statement, water, cell phone or paystub. Bills must have parent's name and Bogota address. 

For renters with a lease (2 requirements)

• 1. Renter w/lease Requirement 1: If you are a renter upload a copy of your current lease. Lease must be for the current year!

  2. Renter w/lease Requirement 2: Please provide 2 bills such as gas, cable, electric, credit card or bank statement, water, cell phone or paystub. Bills must have parent's name and Bogota address. 

For renters without a lease (3 requirements)

• 1. Renter w/o lease Requirement 1: Please upload your notarized and complete affidavit in the form below. (affidavit of landlord form)

     1. print out the form 

     2. filled out by hand 

     3. notarize by the landlord

     4. scan and upload in the form below

  2. Renter w/o lease Requirement 2: Proof of ownership by the landlord in the form of a deed, current tax bill, or current mortgage statement. 

  3. Renter w/o lease Requirement 3: Please provide 2 bills such as gas, cable, electric, credit card or bank statement, water, cell phone or paystub. Bills must have parent's name and Bogota address.
