KEY INFORMATION FOR CDM PATIENTS

GENERAL INFORMATION

Individuals who are being managed by the GP for chronic illness may be eligible for a Chronic Disease Management (CDM) Plan, sometimes referred to as an 'EPC' plan. 

Individuals who are referred by their GP to Bluff Road and North Road Physiotherapy under a CDM plan are eligible for up to 5 physiotherapy sessions. These sessions are, in part, subsidised by Medicare. 

If your GP refers you for physiotherapy under this program, they will send us your referral indicating your treatment needs and how many sessions you will be subsidised for. 

Important: Bluff Road and North Road Physiotherapy must receive your referral prior to your visit for the session to be eligible for the Medicare subsidy. 

PRICING AND PAYMENT

Physiotherapy sessions under the CDM program are part in part by Medicare - Medicare does NOT pay for the full consultation fee. Bluff Road and North Road Physiotherapy do NOT bulk bill CDM sessions. 

The CDM Price Structure is as follows:

Senior Physiotherapist - $105 to be paid on the day of the appointment, get rebated $56, patient will be out of pocket $49.

Associate Physiotherapist - $95 to be paid on the day of the appointment, get rebated $56, patient will be out of pocket $39.

Physiotherapist - $85 to be paid on the day of the appointment, get rebated $56, patient will be out of pocket $29.
