As a new parent or family in our community, you will be asked to complete a few different forms when enrolling your student. Note that ALL Required forms need to be completed under the Genesis Parent Portal. The following represents only the content of those forms for review purposes only.
Physicals - Any new student coming into Bloomingdale must provide proof of a physical.
If you are transferring from another NJ School district and you have had a physical, please provide proof to your school nurse
If you are transferring from out of state or out of the US, you must provide proof of physical that has been done within the last 365 days.
IF you don't have a copy of the physical per the requirements above, you must get a physical for your child. Please print the following form, present it to your doctor for the physical and return the completed version to your school nurse. Universal Health Record

State Required Physicals
Upon enrollment in school. Each school district shall require parents to provide within 30 days of enrollment entry-examination documentation for each student. When a student transfers to another school, the sending school district shall ensure the entry-examination documentation is forwarded to the receiving school district pursuant to N.J.A.C. 6A:16-2.4(d).iii. Students transferring into a New Jersey school from out-of-State or out-of-country may be allowed a 30-day period to obtain entry-examination documentation. Each school district shall notify parents through its website or other means about the importance of obtaining subsequent medical examinations of the student at least once during each developmental stage: at early childhood (preschool through grade three), pre-adolescence (grade four through six), and adolescence (grade seven through 12);
Sports Physical Forms - Most of these forms are to be printed, completed by you or the doctor and submitted to the school nurse.
History Form (Completed by Parent)
Must be completed by the parent regardless of whether a family physician or the school physical conducts the exam
Athlete Physical Evaluation History Form (Completed by Doctor)
Use this form if you have your healthcare provider complete the physical. This form is to be completed by your physician.
Consent for the School Physician complete the physical.
Use this form if you consent to have the school physician complete the physical.
