A note from Maschios - our Food Services Vendor. 

Food Allergies and Special Dietary Need Requests:

If you would like an “Allergen Safe Menu,” “Special Diet Menu,” or Milk Substitution please complete our Medical Statement Request Form for Special Meals and Milk Substitutions. All documentation may also be completed online by visiting https://bit.ly/specialmealrequestform. If you prefer to submit the documentation digitally please email nutrition@maschiofood.com and use the subject line “MEDICAL STATEMENT”. Alternatively, the form may be faxed to (908) 888-2335. Please note that supplemental documentation may be required from your child’s licensed medical provider. Updated medical forms must be submitted to the nutrition department annually as these menus do not rollover year to year. If a “Allergen Safe Menu,” “Special Diet Menu,” or Milk Substitution is not needed, no paperwork is required.

*Notes:

• Safe menus automatically discontinue at the end of each school year

• Waivers or consent forms are no longer needed for children not proceeding with a safe menu

• POS Systems (e.g. Genesis) - Allergy alerts should not be the sole safety measure for students with food allergies as there may be hidden allergens in some menu items. We encourage students with food allergies to be provided with a safe menu.

Carbohydrate Counts and Menus:

You can access your Nutrislice menus in two ways. You can use a web browser (such as Chrome, Firefox, Microsoft Edge, Safari, etc.) and go to https://bloomingdaleschools.nutrislice.com/menu.  Alternatively, you can also access your menus by downloading the Nutrislice app on your mobile device. (Please note the Nutrislice app is not available for use on desktop or laptop computers.)
