Intensive Outpatient Program ASAM 2.1, Partial Hospitallization Program ASAM 2.5
Clinical Director (Addiction Counselor Supervisor):
Oversee program operations and compliance.
Ensure timely assessments, plans, group scheduling.
Must be licensed at independent practice level.
Counselors (SUD licensed staff):
Assessment within 1 week of admission.
Treatment Plan within 5 working days of assessment, signed by client + counselor.
Update plan every 30 days.
Provide required services:
4 group session per week (≤15 per group).
1 individual session every 2 weeks.
Family services as clinically needed.
Documentation: Group notes, individual progress notes, plan updates, discharge summaries.
Case Managers:
Provide care coordination, linkages to community supports.
Documentation: Case management notes, referral follow-ups, crisis linkage documentation.
Clinical Director (Full-time, Board-approved counselor supervisor):
Oversee operations, supervise counselors.
Approve plans, ensure staffing ratios (≤15:1).
Counselors:
Assessment at admission.
Treatment Plan within 5 working days; update every 30 days.
Deliver 20–35 hrs/wk of structured services.
Documentation: Progress notes, treatment plan reviews, discharge summaries.
Psychiatrist:
Conduct initial evaluation at admission; provide ongoing medical oversight.
Supervise medication management.
Documentation: Psychiatric eval, med management notes, treatment plan input.
Registered Nurse:
Complete health/nursing assessment at admission.
Provide med administration, vitals, health monitoring.
Documentation: Nursing notes, med logs.
Therapists (LCPC/LCSW-C/etc.):
Facilitate daily groups, individual therapy, crisis intervention.
Update treatment plan every 6 months (or sooner).
Documentation: Progress notes each session, crisis documentation.
