Sliding Fee Scale Policy
We are committed to providing affordable services to all members of our community. We offer a Sliding Fee Scale (SFS) based on your income and family size to ensure that cost is not a barrier to receiving the care you need. At a minimum, within the policy, the following areas must be addressed:
The purpose of the SFS is a model for payments administered to patients, which are fees for services adjusted depending on an individual’s income. This is set to allow for fairness and address income inequality.
Eligibility for the Sliding Fee Scale is determined based on your household income and size. To apply, please provide proof of income, such as recent pay stubs, tax returns, or any other relevant financial documents.
An explanation of SFS charges based on the Federal Poverty Level Eligibility for the Sliding Fee Scale must be reassessed annually. Applicants must reapply each year by submitting updated documentation.
Applicants denied or disagreed with their discount level can appeal against the decision by submitting a written appeal to Horizon Wellness Services within 14 days of notification. The appeal will be reviewed, and a decision will be made within 7-10 business days.
