This form is for JUNIOR HIGH SCHOOL (GRADE 7) STUDENTS ONLY.
Prepare the following documents before answering:
Copy of Grade 6 Report Card with 1st to 3rd Quarter Grades (front and back) with an average of at least 85%,
LRN or Learner's Refence Number,Â
PWD ID/Medical Diagnosis, if PWD;
Official Receipt of the Admission Exam Fee Payment
AVOID filling out this form MULTIPLE TIMES.
DO NOT FORGET YOUR EMAIL AND PASSWORD. Once your application has been checked and verified, the Admission and Retention Office will send a response to the SAME email that you used in your application.
