PLEASE READ BEFORE FILLING OUT THE FORM TO AVOID PROBLEMS/DELAYS ON YOUR ADMISSION APPLICATION FORM SUBMISSION:

1. Ensure that your INTERNET CONNECTION IS STABLE.

2. This form is for GRADUATE STUDIES STUDENTS ONLY (MASTER'S & DOCTORATE).

3. If you have taken the admission/entrance examination for Graduate Studies in the previous years, DO NOT PROCEED IN FILLING OUT THIS FORM. Contact or visit the Admission and Retention Office for advice.

4. If you have been enrolled in BiPSU School of Graduate Studies before, DO NOT PROCEED IN FILLING OUT THIS FORM. Contact or visit the School of Graduate Studies (SGS) Dean's Office or the Admission and Retention Office for advice.

5. Prepare the following documents before answering:

• Transcript of Records (TOR) or Copy of Grades (COG),

  • for MASTER’S DEGREE: must be a holder of an appropriate and corresponding bachelor’s degree with at least an average rating of 2.0 or its equivalent from any CHED-recognized academic institution of higher learning.

  • for DOCTORATE DEGREE: must be a holder of an appropriate and corresponding master’s degree with at least an average rating of 1.75 or its equivalent from any CHED-recognized academic institution of higher learning and a minimum of 5 years of professional work experience related to the degree being applied for.

• PWD ID/Medical Diagnosis, if PWD;

• Official Receipt of the Admission Exam Fee Payment

6. Select your MOST PREFERRED PROGRAM as your 1st CHOICE. Your application will be processed primarily based on your 1st CHOICE.

7. AVOID filling out this form MULTIPLE TIMES.

8. DO NOT FORGET YOUR EMAIL AND PASSWORD. Once your application has been checked and verified, the Admission and Retention Office will send a response to the SAME email that you used in your application.
