PLEASE READ BEFORE FILLING OUT THE FORM TO AVOID PROBLEMS/DELAYS ON YOUR ADMISSION APPLICATION FORM SUBMISSION:

1. Ensure that your INTERNET CONNECTION IS STABLE.

2. This form is for INCOMING/NEW UNDERGRADUATE STUDENTS AND TRANSFEREES ONLY. Shiftees or students previously enrolled in any BiPSU Undergraduate/College Program or Course should consult with their respective Advisers/School Deans first.

3. Prepare the following documents before answering:

   • Clear Scanned Copy/ Photo of Senior High School Report Card with 1st Quarter Grades (front and back), if SHS student;

   • Clear Scanned Copy/Photo of High School Report Card if Basic Education Curriculum (High School Graduate); (if Report Card is not yet available, you may upload a scanned copy/photo of your current School ID.)

   • Learning Reference Number (LRN) if SHS student;  

   • Clear Scanned Copy/Photo of Certificate of Grades or TOR, if transferee;  (if TOR is not yet available, you may upload a scanned copy/photo of your current School ID.)

   • Clear Scanned Copy/Photo of ALS Certificate if ALS Passer (eligible for College); and

   • Clear Scanned Copy/Photo of PWD ID/Medical Diagnosis, if PWD; and

   • Clear 2x2 ID Picture or a formal or decent Picture/Selfie. (Note: Avoid shirtless pictures, or wearing sleeveless.)

4. Select the Campus and Program that you are MOST CERTAIN TO PURSUE as your 1st CHOICE. Your application will be processed primarily based on your 1st CHOICE. (NOTE: Changing of Campus and Program/Courses AFTER filling out the Application Form is NOT ADVISED.)

5. AVOID filling out this form MULTIPLE TIMES.

6. DO NOT FORGET YOUR EMAIL AND PASSWORD. Once your application has been checked and verified, the Admission and Retention Office will send your examination schedule and other details to the SAME email that you used in your application.
